What Medicare Actually Costs in 2026

Every figure below comes from the official CMS fact sheet for 2026. We have organized it the way you actually experience it: what comes out of your check every month, what you pay when you use care, and the income brackets that quietly decide your premium two years in advance.

Part A: what the hospital side costs when you use it

Most people pay no Part A premium. The costs show up when you are admitted. Per benefit period in 2026:

Skilled nursing facility, per benefit period: $0 for days 1 through 20, then $217 per day for days 21 through 100, then all costs. Notice what Medicare does not do here: it is not long-term care coverage, and day 101 is the cliff. That gap is exactly why the roof layer of the MV Blueprint treats long-term care as its own decision.

Part B: the premium your tax return sets

The standard 2026 Part B premium is $202.90 per month, and the annual Part B deductible is $283. But if your income is above the thresholds, you pay an income-related monthly adjustment amount (IRMAA), and here is the part that surprises people: your 2026 premium is set by your 2024 tax return.

Part D: the same income brackets, applied to drug coverage

Higher incomes also pay a monthly adjustment on top of whatever their drug plan charges:

Why this is a planning page, not just a price list. That two-year lookback means IRMAA is one of the few "taxes" you can see coming and steer around. The income on this year's return sets your premiums two years from now, so the timing of Roth conversions, large withdrawals, and capital gains decides which row of these tables you live in. Cross a threshold by one dollar and you pay the entire higher premium, every month, for a full year, for both spouses. This is the wiring layer of the MV Blueprint doing its quiet work.

Part C premiums vary by plan; compare current offerings at Medicare.gov/plan-compare.
